Step 1 - General
This step presents the following fields:
| Item | Description |

| Vendor | The system fills in the field automatically. |
| Justification* | Required field. Provide space for inserting free text with justification. |
| Reason* | Required field. Features a retractable menu for choosing a registered reason. |
| Governance code | Optional field. Provides space for recording the governance code, if applicable. |
| Next | Button to proceed to the next step. |
Step 2 - Devices
This step presents the following fields:

| Plus icon | Opens a window with the list of devices available for access. |
| Previous | Button to go back to the previous step. |
| Next | Button to proceed to the next step. |
Step 3 - Credentials
This step presents the following fields:

| Previous | Button to go back to the previous step. |
| Next | Button to go to the next stage. |

The section only displays credentials related to previously selected devices.
Step 4 - Access Restriction
This step presents the following fields:

| Access permission period | Section for specifying the start and duration of access. |
| Access permission days | Section for specifying the days of the week on which access will be available. |
| Access permission times | Section to specify the times when access will be available. In addition to the predefined options, it is possible to customize a time window for access. |
| Sessions | Field for specifying a limit on the number of sessions. |
| Previous | Button to go back to the previous step. |
| Next | Button to proceed to the next step. |
Step 5 - Review
This step presents the following fields:
